Location: Astoria Senior Living | Omaha, NE
Pay Range: $70,000 – $75,000 annually + bonus potential
Schedule: Monday–Friday, primarily daytime hours, with flexibility for evenings or weekends as needed

Astoria is a vibrant, 133-resident community offering both assisted living and memory care, and we’re looking for a Health and Wellness Director (HWD) who can bring clinical expertise, strong leadership, and a genuine passion for improving lives.

What You’ll Do

As the Health and Wellness Director, you’ll oversee the nursing and wellness operations at Astoria, ensuring exceptional care for residents while leading, mentoring, and developing your team.

  • Lead by Example – Be visible and engaged: walk the halls, connect with residents, and provide guidance to staff.
  • Resident-Centered Care – Develop, implement, and review personalized service plans to meet residents’ unique needs.
  • Team Leadership – Recruit, train, and mentor caregivers, med aides, and nursing staff while fostering a supportive, high-performing culture.
  • Compliance & Documentation – Ensure accurate records, timely follow-ups, and full compliance with state regulations.
  • Collaboration – Partner closely with families, physicians, and healthcare providers to ensure residents receive coordinated care.
  • Emergency Oversight – Act as the clinical leader during urgent situations and ensure appropriate follow-up procedures are in place.
  • Medication Oversight – Ensure safe and accurate administration of medications and adherence to physician orders.
  • Professional Development – Collaborate with leadership to implement Astoria’s upcoming nursing policies and procedures and integrate them into daily practice.

Top Priorities in the First 90 Days

  • Build strong relationships with residents, families, and your team.
  • Review current care plans, documentation processes, and regulatory readiness.
  • Support efforts to stabilize staffing and strengthen team communication.
  • Partner with leadership to implement Astoria’s updated nursing policies and procedures.

Who We’re Looking For

We’re seeking a collaborative and approachable leader who thrives in a dynamic environment and knows how to balance compassion with accountability.

Must-Haves (Non-Negotiables)

  • Active RN or LPN license in Nebraska (or compact state).
  • Minimum 2–3 years of leadership experience in assisted living, memory care, or a related senior living setting.
  • Strong knowledge of regulatory compliance and care planning best practices.
  • Proven ability to lead and develop teams while maintaining a positive culture.
  • Excellent communication, organizational, and follow-through skills.

Nice-to-Haves (Wish List)

  • Experience with senior living technology platforms and scheduling systems.
  • Background in staff training and development.
  • Familiarity with state surveys and regulatory audits.

Schedule & Role Expectations

  • Typical Hours: Monday–Friday, business hours.
  • Flexibility: Occasional evenings or weekends to support resident or staffing needs.
  • Shift Coverage: Primarily a leadership role; stepping in for urgent coverage is rare but possible.
