Health & Wellness Director

Health and Wellness Director
Location: Holland Farms, Norwalk, IA

Compensation: $80k-$90k plus bonus

Lead with compassion, integrity, and purpose as the Health and Wellness Director at Holland Farms, an Agemark Senior Living community. With 105 apartments (44 in Memory Care and 61 in Assisted Living), we’re home to wonderful residents and families who count on us every day. What You’ll Do

  • Provide leadership with presence: Be visible, approachable, and supportive of staff and residents alike.
  • Care planning & oversight: Develop and maintain service plans, medication management, and assessments in partnership with physicians and families.
  • Build trust through communication: Serve as the primary liaison between residents, families, staff, and healthcare providers.
  • Mentor & develop your team: Train, guide, and encourage wellness staff while maintaining high standards of care.
  • Support when needed: While shift coverage is rare (only 2 in the past year), you’ll model teamwork and step in as needed.
  • Promote resident wellness: Lead initiatives and programming that support preventive health and quality of life.

Schedule & Pay

  • Typical schedule: Monday–Friday, 8–4:30 or 9–5:30, with occasional weekend needs.
  • Salary: $80,000–$90,000, with some flexibility based on experience.

What We’re Looking For

  • RN license (required) in the state of Iowa.
  • Experience in assisted living, memory care, or long-term care.
  • At least 2 years of leadership experience in a healthcare setting.
  • Strong organizational skills with the ability to prioritize and delegate effectively.
  • A leader who is compassionate, respectful, positive, and collaborative.
  • Bonus: Stability in past roles or a clear story for transitions.

What Success Looks Like in the First 90 Days

  • Build strong connections with staff, residents, and families.
  • Prioritize delegation and communication.
  • Stay on top of condition changes, documentation, and the MAR (med/doctor orders).
  • Approach improvements with patience and positivity.
